You do not need any of these to write to me. They just save a round of questions going back and forth.
The plan name as printed
Copy it from the letter or the card exactly as it appears, including the insurer. Similar sounding plans from the same company can behave very differently, and the printed name is what tells them apart.
Your medicine list
Names, strengths and how often you take each one. This is the single most useful thing you can send me, because prescription coverage is where two otherwise similar plans separate most sharply.
Any date on the paperwork
Deadlines printed on a notice, the month your current coverage started, or the date your employer coverage ends. Timing decides which options are open to you, so it changes the whole answer.
